Mara Moore
Preceptor for the Care Connections, orientation, and specialty pharmacy services learning experiences
Mara received her PharmD from the Nesbitt School of Pharmacy at Wilkes University. She completed a PGY1 Pharmacy Residency and a PGY2 Ambulatory Care Pharmacy Residency at Penn Medicine Lancaster General Health. Mara is a Board-Certified Ambulatory Care Pharmacist (BCACP) and currently works at Penn Medicine Lancaster General Health Care Connections and in medication access clinics. Her professional interests include chronic disease management, behavioral health, transitions of care, and precepting. She has been the PGY2 Ambulatory Care Pharmacy Residency Program Director since 2023 and is also a preceptor for both PGY1 and PGY2 programs. Mara is a member of the American Society of Health-System Pharmacists (ASHP) and the Pennsylvania Society of Health-System Pharmacists (PSHP).
Courtney Calamia
Preceptor for the primary care learning experiences
Courtney received her Doctor of Pharmacy (PharmD) degree from Wilkes University, Nesbitt School of Pharmacy. She completed her ambulatory focused post graduate training year-one (PGY1) pharmacy residency at the Wilkes-Barre VA Medical Center. She is a board certified ambulatory care pharmacist (BCACP). Courtney divides her time between two family medicine practice sites: Twin Rose and Lincoln. Her interests include diabetes, anticoagulation stewardship and the importance of continuity of care.
Chelsea Cunningham
Preceptor for the anticoagulation learning experience
Chelsea received her PharmD from the University of the Sciences, Philadelphia College of Pharmacy. She completed a pharmacy practice residency at Penn Medicine Lancaster General Health. After her residency, Chelsea joined the organization as an ambulatory pharmacist clinician and is a board-certified ambulatory care pharmacist (BCACP). She primarily practices at the Crooked Oak Pharmacy Services Anticoagulation Clinic. Her interests include patient education and precepting pharmacy learners.
Andrea Dooley-Wood
Preceptor for the practice management learning experience
Andrea received her PharmD from the University of Rhode Island, College of Pharmacy. She completed a primary care PGY1 pharmacy residency at the Providence Veterans Affairs Medical Center in RI. Andrea is a board-certified ambulatory care pharmacist (BCACP). Her current position is Clinical Manager of Ambulatory Pharmacy Services. Her professional interests include caring for patients with cardiovascular diseases, population health, specialty pharmacy and staff development.
Matthew Eberts
Matt is the Director of Pharmacy & IV Services for Penn Medicine Lancaster General Health. He is a past president of the Pennsylvania Society of Health System Pharmacists (PSHP) and Western Chapter of the PSHP. As a member of PSHP, Matt is the past chair of the Practice Management Section’s Education Steering Committee. He is also a past member of ASHP’s Commission on Affiliate Relations. At LG Health, he is responsible for pharmacy operations at Penn Medicine Lancaster General Hospital, Penn Medicine Lancaster General Health Women & Babies Hospital, affiliated cancer and infusion centers, community pharmacies, and ambulatory pharmacy practices. Matt has published and presented nationally on a variety of subjects including finance, employee satisfaction and retention, departmental communications, strategy, IV preparation, and FDA issued Risk Evaluation and Mitigation Strategies. He received his PharmD from Duquesne University, an MBA from Robert Morris University, completed Six Sigma Green Belt Training from Motorola University, was awarded the Jewish Health Care Foundation’s Perfecting Patient Care Grant and is a graduate of the Pharmacy Leadership Institute at Boston University.
Erin Fleming
Preceptor for the primary care and endocrinology learning experiences
Erin graduated with her PharmD from the University of Maryland and then completed a PGY1 pharmacy residency at LG Health. Prior to obtaining her PharmD, she received her BA in Biology from Hood College and then completed her PhD from The Johns Hopkins University in the department of biology, cell, molecular, developmental biology and biophysics in which she focused on the development of small molecular inhibitors for HIV. Erin works at the following outpatient practices: Crooked Oak, Internal Medicine, Diabetes and Endocrinology. She is licensed to administer injectable medications, is a certified Medication Therapy Management provider, and is also a board certified ambulatory care pharmacist (BCACP). She enjoys managing diabetes, transitions of care and hypertension. Her interests include teaching pharmacy learners and helping individuals navigate the healthcare system to obtain the most appropriate and affordable drug therapies.
Greta L. Kemmer
Preceptor for the anticoagulation and service learning experiences
Greta graduated with her PharmD from the University of Florida College Of Pharmacy. Prior to her doctoral program, Greta received her BS in Pharmacy from the Philadelphia College of Pharmacy and Science and practiced 14 years as a community pharmacist, dedicating her time to the development of patient care initiatives. Greta is a BCACP. Greta is the lead pharmacist at the Norlanco Pharmacy Services Clinic where she provides anticoagulation management services and collaborative drug therapy management services for five family medicine practices. Greta is licensed to administer injectable medications and is a certified Medication Therapy Management provider. Her interests include anticoagulation stewardship, supporting and facilitating the transitions of care process, and leveraging positive patient relationships to improve continuity and quality of patient care.
Nafisa Khan
Preceptor for the preceptor training program and teaching certificate learning experiences
Nafisa received her PharmD from St. John’s University. She completed a PGY1 pharmacy residency at St. Luke’s-Roosevelt Hospital. Prior to joining LG Health, Nafisa held various clinical pharmacy positions including the Northport Veterans Affairs Medical Center and Philadelphia Prison. She works at various outpatient practices, including Norlanco, Mount Joy and Sycamore Square. Her interests include chronic disease education, precepting future pharmacy graduates and collaborating with various services within the organization to align quality goals.
Kathy Makkar
Preceptor for the cardiovascular diseases and anticoagulation learning experiences
Kathy received her PharmD from Philadelphia College of Pharmacy, University of the Sciences. She completed a PGY1 pharmacy residency at Mercy Suburban Hospital followed by a PGY2 cardiology residency at University of the Sciences. Kathy holds board certification in cardiology pharmacy and pharmacotherapy. Kathy is actively involved as a cardiology clinical pharmacy specialist, providing a clinical focus on the CHF and mechanical circulatory support patient populations. Her main interests include advanced heart failure and mechanical circulatory support.
Hien Nguyen
Preceptor for the geriatrics and rheumatology learning experiences
Hien received her PharmD from the Philadelphia College of Pharmacy, University of the Sciences in 2017. She completed a PGY1 Pharmacy Residency at Penn Medicine Lancaster General Health and a PGY2 Geriatrics and Academia Pharmacy Residency at Mountain Area Health Education Center (MAHEC) and University of North Carolina Eshelman School of Pharmacy. She is a Board-Certified Geriatric Pharmacist (BCGP). Hien is an Ambulatory Pharmacist Clinician and works at the Penn Medicine Lancaster General Health Physicians Geriatrics, along with providing medication access services for arthritis and rheumatology patients. Her areas of interest include osteoporosis/bone health, deprescribing/appropriate prescribing, and mentoring/precepting future generations of pharmacists. Hien precepts both PGY1 and PGY2 pharmacy residency programs and serves as a faculty member for the Geriatrics Medicine Fellowship at LGH. She is an active member of PSHP, most recently completing her Immediate Past President role in 2023 for the Southeastern Chapter; along with holding active membership with the American Geriatrics Society (AGS) and American Society of Health Systems Pharmacists (ASHP).
Michelle Link Patterson
Preceptor for the primary care, family medicine education, and presentations and precepting learning experiences
Michelle received her PharmD from Thomas Jefferson University, Jefferson College of Pharmacy. She worked as a community pharmacist for 3 years before completing a PGY1 Pharmacy Residency at Capital Health in Trenton, NJ. She then went on to complete a PGY2 Ambulatory Care Pharmacy Residency at Temple University School of Pharmacy in Philadelphia, PA. After residency, Michelle worked for Penn Medicine University of Pennsylvania Health System before transferring to Penn Medicine Lancaster General Health in 2021. Michelle is a board-certified ambulatory care pharmacist (BCACP) and a Certified Diabetes Care and Education Specialist (CDCES). She is involved nationally with ASHP and ACCP. Michelle’s practice sites are Walter L. Aument Family Health Center and Family Medicine Buck. Her interests include transitions of care, preventive healthcare, and diabetes management.
Nga Pham
Preceptor for the hematology/oncology elective learning experience
Nga Pham received her PharmD from Temple University School of Pharmacy. She completed an Ambulatory Care Pharmacy Practice residency at Harbor Health Services, Inc. (HHSI) in Boston, Massachusetts. After residency, Nga joined the faculty at Northeastern University with a clinical practice site at HHSI. She joined Lancaster General Health in 2013, where she serves as a clinical oncology pharmacist.
Kunjan Shah
Preceptor for the Infectious Diseases/ HIV-AIDS elective learning experience
Kunjan Shah received his PharmD from Mercer University's College of Pharmacy in 2021. He then completed his PGY1 Pharmacy Residency at Wellstar Atlanta Medical Center (Atlanta, GA) followed by a specialty pharmacy residency in Infectious Diseases at UF Health Shands Hospital (Gainesville, FL). Following residency training, Dr. Shah joined Penn Medicine Lancaster General Hospital as an Infectious Diseases Clinical Pharmacy Specialist. His primary areas of interest are antimicrobial and diagnostic stewardship, therapeutic drug monitoring, and gram-negative resistance.
Jennie Smith
Preceptor for the population health learning experience
Jennie Smith received her PharmD from Wilkes University, Nesbitt School of Pharmacy. After graduation, Jennie worked briefly as a staff pharmacist for CVS Health and then accepted a position with Geisinger-Holy Spirit as a clinical pharmacist specializing in pain management and primary care. Jennie then accepted a position with Penn Medicine Lancaster General Health as an ambulatory pharmacist clinician. Jennie is a board certified ambulatory care pharmacist (BCACP). She works at Penn Medicine Lancaster General Health Physicians Family Medicine Susquehanna, Norlanco Pharmacy Services Clinic, and population health management clinic. Her professional areas of interest include population health and diabetes management.
Katlyn Wood
Preceptor for the pain management elective learning experience
Katlyn Wood received a PharmD from Notre Dame of Maryland University School of Pharmacy and completed her pharmacy practice residency at Lancaster General Health. As a Clinical Pharmacy Specialist in pain management, she feels honored to be able to offer assistance with pharmacologic regimens during some of the most painful experiences in patients' lives. She co-leads the Pain Management and Opioid Stewardship Committee at PMLGH, is involved in a number of other committees related to pain and addiction, and has previously served as President of the PSHP South East Chapter. Dr. Wood is board-certified in pharmacotherapy and her passions include starting patients on medication for opioid use disorder, transitions of care, patient education, and teaching.
